Custom Member Goal Plans Created By Clinical Team

Overview

Goal plans allow for our Clinical team to scale support for patients more effectively since they’ll be able to create templates for general journey fits and modify a template to suit a specific patient for personalized 1-on-1 care. 

This will also enable clinical to deliver a more intentional, curated, and holistic experience. Once a goal plan is added to a patient’s journey through Compass, goal plans will enable a way to tie our care navigation services to a member’s UX on the consumer platform.

I worked closely with the product manager, engineers, and most importantly our internal clinical team to iterate on design until we got to the MVP designs shown below.

Goals

Goal plans will enable Jasper care navigators to measure the improvement we bring to our members’ lives. With this in place, there is an infrastructure to determine a member’s baseline of wellbeing, confidence, and readiness.

  • Build an infrastructure for goal plans that is FHIR friendly and compatible with existing features like tasks and questionnaires

  • Create and manage goal plan templates

  • Customize and associate goal plans with a member

  • Demonstrate at a quantifiable level how we improve member outcomes

 

MVP Design

Before Compass goal plans, care navigators would share forms and worksheets with members via Sharepoint. To bring that into Compass, goal plans needed to do the following:

  • Add custom tasks to a goal plan tailored to a specific member

  • Assign a goal plan to a member to provide a structured path for their cancer journey

  • View all goal plans for a specific member

  • Care navigators can view a member’s progress on a specific goal plan

 
 

Future State

Originally, the goal was to make goal plans a lot more robust (pictured right), but that would require 6-8 months of work, and that was not time the clinical team was willing to wait. Unfortunately, I had designed the robust version first. Then, we needed to scale back to the version you saw above.

The future state allows a care navigator to:

  • Create and maintain goal plan templates to have a scalable starting point for creating individualized goal plans for members

  • Search and filter based on the particular type of activity they might think is needed for the member’s cancer journey

  • Add, re-order, and edit both new and already existing activities as needed

 

Prototype

I partnered with the head of clinical, who provided the sample content, to build a prototype showcasing a template for advanced care planning on Compass, as well as what the experience would look like on the consumer app. 

Advanced Care Planning is a process enabling members to make plans for their future health care, such as an emergency or end of life.

The goal of the prototype is to create an advanced care plan for Diane.
